Here
are some of the services that we can offer you:
Bereavement Care Community
bereavement counselors provide individual bereavement support to surviving
loved ones for 13 months following the patients death. In addition
we offer Adult and Childrens support groups which are open to the
public.
In addition to providing comprehensive, compassionate care for the incurably ill and their families, Community Hospice and Palliative Care (Community) is committed to increasing public awareness and understanding of various health related topics. Members of the Community Speakers Bureau are available to accommodate small and large groups for day and evening presentations. The following topics are offered free-of-charge. HOSPICE... A SPECIAL KIND OF CARING - Hospice is choosing life over death within the confines of an incurable illness. Community provides a comprehensive program which enables the patient to live each day in the peace and comfort they deserve. Our team concept places an emphasis on palliative care (state-of-the-art relief of pain and other distressing symptoms related to the illness). Community believes the quality of life and dignity of the individual become the primary concern when physical healing is no longer achievable. By providing competent, caring attention to both physical and emotional needs, Community helps our patients live every day to the fullest... COPING WITH GRIEF AND LOSS - One of life's most devastating experiences is the death of a loved one. Grief that accompanies loss affects us physically, emotionally and spiritually. It is said that time heals all wounds. However, time alone does not heal our broken hearts and battered spirit. We need to make a conscious decision to express our sorrow; to talk about our thoughts and feelings. Community grief recovery programs help surviving loved ones learn about how to face the challenge of grieving and how to grow from the experience. ADVANCE MEDICAL DIRECTIVES...ALL PATIENTS HAVE RIGHTS - Advance Medical Directives protect the patient's rights, give the physician guidelines and help families avoid making difficult decisions. Advance directives are essential in protecting the right to accept or refuse medical care. If a patient becomes unable to communicate his/her medical choices, a designated surrogate can carry out the patient's health care wishes. Durable Power of Attorney for health care decision making, Living Wills, and Do Not Resuscitate Directives will be described and compared. OTHER TOPICS AVAILABLE:
