Our complete healthcare team's focus is quality of life and care for the whole person - physically, emotionally and spiritually:

Physician - either your own attending physician or our Medical Director will
direct the plan of care tailored to meet your specific needs.

Nurse - Specially trained in pain and symptom management utilizing the latest
products, technology and equipment. The Community nurses will:
  • coordinate your care based on the goals you establish
  • provide education to you and your caregivers on care, medications and disease process
  • supervise the home health aide
  • respond to your questions and needs 24 hours a day 7 days a week.
Social Worker - Community social workers assist patients and families with:
  • identifying solutions to problems
  • education and linkage to community resources
  • counseling regarding coping and grief issues
  • education related to life closure tasks such as wills, medical durable power of attorney
  • mediation as needed during times of crisis and conflict
Home Health Aides - Community aides provide:
  • personal care
  • light housekeeping in patient care areas
Volunteers - Community volunteers are specially trained and provide services
such as:
  • respite care
  • errand running
  • hair dressing
  • patient companionship
  • feeding and personal care
Spiritual Care - Community spiritual care staff and spiritual volunteers
provide:
  • counseling and emotional support in accordance with the patient & family's belief system
  • prayer, meditation and sacraments
As needed services:
  • Physical therapy
  • Occupational therapy
  • Speech therapy
  • Nutrition counseling
